One of the best reasons to have a Health Care Proxy (HCP) is to give yourself and your family a plan for what to do in an emergency situation. If at any point you need someone to speak for you medically a HCP must have been filled out beforehand. The paperwork is a simple one page/two sided form that must have two witnesses to your signature and they must sign it as well. No lawyers or notaries needed.
